All C and H files now (should) feature the proper project curl source
code header, which includes basic info, a copyright statement and some
basic disclaimers.

Better cope with a failed or unavailable ftruncate().
...
Added HAVE_FTRUNCATE to all the static config-*.h files on the assumption
that all those systems provide it.
