Source cleanups. The major one being that we now _always_ use a Curl_addrinfo
linked list for name resolved data, even on hosts/systems with only IPv4
stacks as this simplifies a lot of code.

Curl_GetFTPResponse() takes a different set of parameters and now return a
proper CURLcode. The default timeout for reading one response is now also
possible to change while running.
