60bd22620a mprintf: Replaced internal usage of FORMAT_OFF_T and FORMAT_OFF_TU
Following commit 0aafd77fa4, replaced the internal usage of
FORMAT_OFF_T and FORMAT_OFF_TU with the external versions that we
expect API programmers to use.

This negates the need for separate definitions which were subtly
different under different platforms/compilers.

e79535bc5e SessionHandle: the protocol specific pointer is now a void *
All protocol handler structs are now opaque (void *) in the
SessionHandle struct and moved in the request-specific sub-struct
'SingleRequest'. The intension is to keep the protocol specific
knowledge in their own dedicated source files [protocol].c etc.

There's some "leakage" where this policy is violated, to be addressed at
a later point in time.

4ad8e142da urldata: clean up the use of the protocol specific structs
1 - always allocate the struct in protocol->setup_connection. Some
protocol handlers had to get this function added.

2 - always free at the end of a request. This is also an attempt to keep
less memory in the handle after it is completed.

5a053ffe80 build: fix circular header inclusion with other packages
This commit renames lib/setup.h to lib/curl_setup.h and
renames lib/setup_once.h to lib/curl_setup_once.h.

Removes the need and usage of a header inclusion guard foreign
to libcurl. [1]

Removes the need and presence of an alarming notice we carried
in old setup_once.h [2]

1 - lib/setup_once.h used __SETUP_ONCE_H macro as header inclusion guard
    up to commit ec691ca3 which changed this to HEADER_CURL_SETUP_ONCE_H,
    this single inclusion guard is enough to ensure that inclusion of
    lib/setup_once.h done from lib/setup.h is only done once.

    Additionally lib/setup.h has always used __SETUP_ONCE_H macro to
    protect inclusion of setup_once.h even after commit ec691ca3, this
    was to avoid a circular header inclusion triggered when building a
    c-ares enabled version with c-ares sources available which also has
    a setup_once.h header. Commit ec691ca3 exposes the real nature of
    __SETUP_ONCE_H usage in lib/setup.h, it is a header inclusion guard
    foreign to libcurl belonging to c-ares's setup_once.h

    The renaming this commit does, fixes the circular header inclusion,
    and as such removes the need and usage of a header inclusion guard
    foreign to libcurl. Macro __SETUP_ONCE_H no longer used in libcurl.

2 - Due to the circular interdependency of old lib/setup_once.h and the
    c-ares setup_once.h header, old file lib/setup_once.h has carried
    back from 2006 up to now days an alarming and prominent notice about
    the need of keeping libcurl's and c-ares's setup_once.h in sync.

    Given that this commit fixes the circular interdependency, the need
    and presence of mentioned notice is removed.

    All mentioned interdependencies come back from now old days when
    the c-ares project lived inside a curl subdirectory. This commit
    removes last traces of such fact.

2c905fd1f8 query-part: ignore the URI part for given protocols
By setting PROTOPT_NOURLQUERY in the protocol handler struct, the
protocol will get the "query part" of the URL cut off before the data is
handled by the protocol-specific code. This makes libcurl adhere to
RFC3986 section 2.2.

Test 1220 is added to verify a file:// URL with query-part.

d7934b8bd4 curl_multi_fdset: correct fdset with FTP PORT use
After a PORT has been issued, and the multi handle would switch to the
CURLM_STATE_DO_MORE state (which is unique for FTP), libcurl would
return the wrong fdset to wait for when curl_multi_fdset() is
called. The code would blindly assume that it was waiting for a connect
of the second connection, while that isn't true immediately after the
PORT command.

Also, the function multi.c:domore_getsock() was highly FTP-centric and
therefore ugly to keep in protocol-agnostic code. I solved this problem
by introducing a new function pointer in the Curl_handler struct called
domore_getsock() which is only called during the DOMORE state for
protocols that set that pointer.

The new ftp.c:ftp_domore_getsock() function now returns fdset info about
the control connection's command/response handling while such a state is
in use, and goes over to waiting for a writable second connection first
once the commands are done.

The original problem could be seen by running test 525 and checking the
time stamps in the FTP server log. I can verify that this fix at least
fixes this problem.

13b64d7558 protocols: use CURLPROTO_ internally
The PROT_* set of internal defines for the protocols is no longer
used. We now use the same bits internally as we have defined in the
public header using the CURLPROTO_ prefix. This is for simplicity and
because the PROT_* prefix was already used duplicated internally for a
set of KRB4 values.

The PROTOPT_* defines were moved up to just below the struct definition
within which they are used.

8831000bc0 protocol handler: added flags field
The protocol handler struct got a 'flags' field for special information
and characteristics of the given protocol.

This now enables us to move away central protocol information such as
CLOSEACTION and DUALCHANNEL from single defines in a central place, out
to each protocol's definition. It also made us stop abusing the protocol
field for other info than the protocol, and we could start cleaning up
other protocol-specific things by adding flags bits to set in the
handler struct.

The "protocol" field connectdata struct was removed as well and the code
now refers directly to the conn->handler->protocol field instead. To
make things work properly, the code now always store a conn->given
pointer that points out the original handler struct so that the code can
learn details from the original protocol even if conn->handler is
modified along the way - for example when switching to go over a HTTP
proxy.

54d9f060b4 Curl_gmtime: added a portable gmtime
Instead of polluting many places with #ifdefs, we create a single place
for this function, and also check return code properly so that a NULL
pointer returned won't cause problems.

07416b61e3 - Using the libssh2 0.19 function libssh2_session_block_directions(), libcurl
now has an improved ability to do right when the multi interface (both
  "regular" and multi_socket) is used for SCP and SFTP transfers. This should
  result in (much) less busy-loop situations and thus less CPU usage with no
  speed loss.

ad638da2c2 Library internal only C preprocessor macros FORMAT_OFF_T and FORMAT_OFF_TU
remain in use as internal curl_off_t print formatting strings for the internal
*printf functions which still cannot handle print formatting string directives
such as "I64d", "I64u", and others available on MSVC, MinGW, Intel's ICC, and
other DOS/Windows compilers.

This reverts previous commit part which did:

b620e62f0f - Dmitry Kurochkin moved several struct fields from the connectdata struct to
the SingleRequest one to make pipelining better. It is a bit tricky to keep
  them in the right place, to keep things related to the actual request or to
  the actual connection in the right place.

13648f8ccd struct HandleData is now called struct SingleRequest, and is only for data that
is inited at the start of the DO action. I removed the Curl_transfer_keeper
struct completely, and I had to move out a few struct members (that had to
be set before DO or used after DONE) to the UrlState struct. The SingleRequest
struct is accessed with SessionHandle->req.

One of the biggest reasons for doing this was the bunch of duplicate struct
members in HandleData and Curl_transfer_keeper since it was really messy to
keep track of two variables with the same name and basically the same purpose!

50feea3eef Rearranged code and changed Curl_readwrite_init() and Curl_pre_readwrite() into
do_init() and do_complete() which now are called first and last in the DO
function. It simplified the flow in multi.c and the functions got more
sensible names!

5b358603bd Michal Marek forwarded the bug report
https://bugzilla.novell.com/show_bug.cgi?id=332917 about a HTTP redirect to
FTP that caused memory havoc. His work together with my efforts created two
fixes:

#1 - FTP::file was moved to struct ftp_conn, because is has to be dealt with
     at connection cleanup, at which time the struct HandleData could be
     used by another connection.
     Also, the unused char *urlpath member is removed from struct FTP.

#2 - provide a Curl_reset_reqproto() function that frees
     data->reqdata.proto.* on connection setup if needed (that is if the
     SessionHandle was used by a different connection).

91fd2c3bcd Bug report #1779751 (http://curl.haxx.se/bug/view.cgi?id=1779751) pointed
out that doing first a file:// upload and then an FTP upload crashed libcurl
or at best caused furious valgrind complaints. Fixed now by making sure we
free and clear the file-specific struct properly when done with it.

aed0cc6f2a Using fdopen() is a more correct way to implement the CURLOPT_NEW_FILE_PREMS
file.c, but the debug interface was missing.  This adds the routines needed
to make the memory debuging work for fdopen().

4cd7f85410 Add two new options for the SFTP/SCP/FILE protocols: CURLOPT_NEW_FILE_PERMS
and CURLOPT_NEW_DIRECTORY_PERMS.  These control the premissions for files
and directories created on the remote server.  CURLOPT_NEW_FILE_PERMS
defaults to 0644 and CURLOPT_NEW_DIRECTORY_PERMS defaults to 0755

385e612fa5 - Armel Asselin improved libcurl to behave a lot better when an easy handle
doing an FTP transfer is removed from a multi handle before completion. The
  fix also fixed the "alive counter" to be correct on "premature removal" for
  all protocols.

e85e30546c Roland Blom filed bug report #1481217
(http://curl.haxx.se/bug/view.cgi?id=1481217), with follow-ups by Michele Bini
and David Byron. libcurl previously wrongly used GetLastError() on windows to
get error details after socket-related function calls, when it really should
use WSAGetLastError() instead.

When changing to this, the former function Curl_ourerrno() is now instead
called Curl_sockerrno() as it is necessary to only use it to get errno from
socket-related functions as otherwise it won't work as intended on Windows.

4f5a6a33b4 moved the lseek() and stat() magic defines to setup.h and now take advantage
of struct_stat in formdata.c as well, to support formpost uploads of large
