multi: fix the transfer hashes in the socket hash entries
- The transfer hashes weren't using the correct keys so removing entries
  failed.

- Simplified the iteration logic over transfers sharing the same socket and
  they now simply are set to expire and thus get handled in the "regular"
  timer loop instead.

cbae73e1dd llist: no longer uses malloc
The 'list element' struct now has to be within the data that is being
added to the list. Removes 16.6% (tiny) mallocs from a simple HTTP
transfer. (96 => 80)

Also removed return codes since the llist functions can't fail now.

Test 1300 updated accordingly.

4f2e348f9b hash: move key into hash struct to reduce mallocs
This removes one tiny malloc for each hash struct allocated. In a simple
case like "curl localhost", this save three mallocs.

e60fe20fdf llist: replace Curl_llist_alloc with Curl_llist_init
No longer allocate the curl_llist head struct for lists separately.

Removes 17 (15%) tiny allocations in a normal "curl localhost" invoke.

b419e7ae0c hostcache: made all host caches use structs, not pointers
This avoids unnecessary dynamic allocs and as this also removed the last
users of *hash_alloc() and *hash_destroy(), those two functions are now
removed.

0f147887b0 Multiple pipelines and limiting the number of connections.
Introducing a number of options to the multi interface that
allows for multiple pipelines to the same host, in order to
optimize the balance between the penalty for opening new
connections and the potential pipelining latency.

Two new options for limiting the number of connections:

CURLMOPT_MAX_HOST_CONNECTIONS - Limits the number of running connections
to the same host. When adding a handle that exceeds this limit,
that handle will be put in a pending state until another handle is
finished, so we can reuse the connection.

CURLMOPT_MAX_TOTAL_CONNECTIONS - Limits the number of connections in total.
When adding a handle that exceeds this limit,
that handle will be put in a pending state until another handle is
finished. The free connection will then be reused, if possible, or
closed if the pending handle can't reuse it.

Several new options for pipelining:

CURLMOPT_MAX_PIPELINE_LENGTH - Limits the pipeling length. If a
pipeline is "full" when a connection is to be reused, a new connection
will be opened if the CURLMOPT_MAX_xxx_CONNECTIONS limits allow it.
If not, the handle will be put in a pending state until a connection is
ready (either free or a pipe got shorter).

CURLMOPT_CONTENT_LENGTH_PENALTY_SIZE - A pipelined connection will not
be reused if it is currently processing a transfer with a content
length that is larger than this.

CURLMOPT_CHUNK_LENGTH_PENALTY_SIZE - A pipelined connection will not
be reused if it is currently processing a chunk larger than this.

CURLMOPT_PIPELINING_SITE_BL - A blacklist of hosts that don't allow
pipelining.

CURLMOPT_PIPELINING_SERVER_BL - A blacklist of server types that don't allow
pipelining.

5a053ffe80 build: fix circular header inclusion with other packages
This commit renames lib/setup.h to lib/curl_setup.h and
renames lib/setup_once.h to lib/curl_setup_once.h.

Removes the need and usage of a header inclusion guard foreign
to libcurl. [1]

Removes the need and presence of an alarming notice we carried
in old setup_once.h [2]

----------------------------------------

1 - lib/setup_once.h used __SETUP_ONCE_H macro as header inclusion guard
    up to commit ec691ca3 which changed this to HEADER_CURL_SETUP_ONCE_H,
    this single inclusion guard is enough to ensure that inclusion of
    lib/setup_once.h done from lib/setup.h is only done once.

    Additionally lib/setup.h has always used __SETUP_ONCE_H macro to
    protect inclusion of setup_once.h even after commit ec691ca3, this
    was to avoid a circular header inclusion triggered when building a
    c-ares enabled version with c-ares sources available which also has
    a setup_once.h header. Commit ec691ca3 exposes the real nature of
    __SETUP_ONCE_H usage in lib/setup.h, it is a header inclusion guard
    foreign to libcurl belonging to c-ares's setup_once.h

    The renaming this commit does, fixes the circular header inclusion,
    and as such removes the need and usage of a header inclusion guard
    foreign to libcurl. Macro __SETUP_ONCE_H no longer used in libcurl.

2 - Due to the circular interdependency of old lib/setup_once.h and the
    c-ares setup_once.h header, old file lib/setup_once.h has carried
    back from 2006 up to now days an alarming and prominent notice about
    the need of keeping libcurl's and c-ares's setup_once.h in sync.

    Given that this commit fixes the circular interdependency, the need
    and presence of mentioned notice is removed.

    All mentioned interdependencies come back from now old days when
    the c-ares project lived inside a curl subdirectory. This commit
    removes last traces of such fact.

d021f2e8a0 Introducing a new persistent connection caching system using "bundles".
A bundle is a list of all persistent connections to the same host.
The connection cache consists of a hash of bundles, with the
hostname as the key.
The benefits may not be obvious, but they are two:

1) Faster search for connections to reuse, since the hash
   lookup only finds connections to the host in question.
2) It lays out the groundworks for an upcoming patch,
   which will introduce multiple HTTP pipelines.

This patch also removes the awkward list of "closure handles",
which were needed to send QUIT commands to the FTP server
when closing a connection.
Now we allocate a separate closure handle and use that
one to close all connections.

This has been tested in a live system for a few weeks, and of
course passes the test suite.

62f0f5571d Robert Iakobashvili re-arranged the internal hash code to work with a custom
hash function for different hashes, and also expanded the default size for
the socket hash table used in multi handles to greatly enhance speed when
very many connections are added and the socket API is used.

c39858aac0 Source cleanups. The major one being that we now _always_ use a Curl_addrinfo
linked list for name resolved data, even on hosts/systems with only IPv4
stacks as this simplifies a lot of code.

14597475b1 Jeff Pohlmeyer did some marvelous debugging to track this one down. We MUST
NOT free the existing hash entry when we try to add a new one that matches
an existing entry. We now instead free the new one, and make the parent
function use the old entry's struct instead.

2cff251863 Curl_resolv() now returns a different struct, and it contains a reference
counter so that the caller needs to decrease that counter when done with
the returned data.

If compiled with MALLOCDEBUG I've added some extra checking that the counter
is decreased before a handle is closed etc.

8d7f402efb Make cach'ing work with threads now, there are now three cases:
- Use a global dns cache (via setting the tentatively named,
    CURLOPT_DNS_USE_GLOBAL_CACHE option to true)
    - Use a per-handle dns cache, by default
    - Use a pooled dns cache when in the "multi" interface
